ACAS is a multi-disciplinary team who assist older people and their carers to identify what kind of care will best meet their needs when they are no longer able to manage at home without assistance. ACAS provides a comprehensive medically based assessment of care needs including information, advice and referral to a range of suitable care options. 

For an older person to access Commonwealth funded residential care, residential respite, Home Care Packages or Transition Care, they must first be assessed as eligible by an ACAS. Transition Care is aimed at improving independence and confidence immediately after a hospital stay. 

Bendigo ACAS provides services for the Loddon Mallee Region, except the Mildura area.

Referral Process 
The preferred referral method is via My Aged Care (MAC) contact centre or online. Ph. 1800 200 422. 

Basic demographic information is required including full name, date of birth, contact details including address and phone number, gender, GP details, consultant details if applicable and the medical condition/reason for referral.

Referrals are prioritised depending on the circumstances of the individual. 
ACAS office is co-located with Community Allied Health Services at the Havlin St East campus. Outreach offices are situated at Echuca, Castlemaine and Swan Hill.

Single Room - $700 p/week
Double/Twin Room - $990 p/week
These prices are all inclusive.

Respite is also available - separate fees.

Low Level Aged Care Residential Accommodation - offering Permanent, Short and Long Term care.
Also offers Respite and Convalescent Care.

All meals, morning and afternoon teas and supper are provided, medication management, assistance with personal care requirements, laundry and room servicing and daily activities and recreation program.

Aged 50 and over, ambulant

Other Details
Approved provider of funded respite and convalescent care for Department of Veteran Affairs (DVA) and some Private Health Insurance Companies.

Aged Care Assessment Services (ACAS) helps older people and their carers to identify what kind of care will best meet their needs when they are no longer able to manage at home without assistance. ACAS assesses and approves older people for aged care services, including: home care packages; transition care – for people immediately following a hospital stay; residential respite care; residential aged care. Castlemaine Health

Mount Alexander shire Council provides a Living at Home assessment to persons in their own home. The person is actively involved in the assessment process , working with the assessment officer to develop a care plan that identifies  their needs and abilities and outlines goals for the person to work towards to remain active and independent at home . The process also supports carers and in their caring role and can involve family and significant others. Consideration is made to people  with special needs – financially disadvantaged ,CALD ( culturally and linguistic  diverse),  people of an Aboriginal or Torres Strait Islander background, people with dementia  and  people who are GLBTI as part of the assessment process. 
Services are provided to achieve the persons goals and referrals are made  for other support such as ACAS ,  Occupational Therapy, District Nursing , Podiatry , Social Support and other allied health. Services are provided with other agencies using a collaborative and coordinated team approach. The assessment service monitors individuals  progress and will review the person as regularly or as needs change.

Loddon Shire Council - Community Services

ASSESSMENTS
The Council's Assessment Officer assesses and monitors the needs of all people referred to the service. This is to ensure that customers receive the most appropriate services and their situation continues to warrant the use of services. The Assessment Officer will refer and liaise where appropriate and maintain and foster maximum customer independence.

ASSESSMENT OFFICER'S RESPONSIBILITIES
Conduct assessments using the Department of Human Services, Service Co-Ordination Tool (SCTT). The SCTT tool is used for referral purposes this information is important and is hoped that duplication of information by other professionals will be minimized, thus being less intrusive for the customer.

Assessment Officer will regularly review the needs and circumstances of HACC customers and initiate increase or terminate services to customer as necessary. Inform customers of Council's service parameters and of customer's rights and responsibilities. Refer customers to other professionals as required. Upon completion of the assessment the customer will be issued with a copy of the assessment.  

About Home and Community Care (HACC) services
The Home and Community Care (HACC) Program provides a range of basic support services to frail older people and people with disabilities who are experiencing difficulties in managing daily tasks, but who wish to remain living at home.  The program also supports their carers and families.

Services include: Home Care, Personal Care, Respite, Home Maintenance, Meals on Wheels, Social Support and Planned Activities.

The HACC program targets its services to those who have the greatest need for them and/or the greatest capacity to benefit from them.

We will assess your needs and provide you with information about your choices.  After the assessment, the HACC Assessment Officer then decides what services can be allocated to you.

It is important to understand that often there are more people wanting services that there are services available.  It is our responsibility to make sure services are allocated fairly and on the basis of need.

Central Goldfields Shire Council 'Caring For Our Community' is committed to providing a professional high quality service by responding to and assisting with, customer's identified needs.  'Caring For Our Community' is also committed to maintaining the maximum level of independence of older people and people with disabilities to prevent premature or inappropriate admission to institutionalised care.  To provide services to assist the aged and disabled to remain in their homes safely.

Fees:  A nominal fee is charged for HACC services.

To promote independence of individuals by the identification of appropriate support services, developing a care plan and monitoring the care plan for effectiveness. Working with the individuals strengths.

Murchison Community Care

A Living at Home Assessment is a person-centred assessment of the person and their carer's needs, which leads to a care plan and individualised service responses. A Living at Home Assessment is a broad, holistic needs based assessment and can include: a service specific assessment and care plan for the range of HACC services delivered by the assessing organisation; a referral action plan to both HACC and non-HACC services that cannot be met by the assessing organisation.
