ACAS is a multi-disciplinary team who assist older people and their carers to identify what kind of care will best meet their needs when they are no longer able to manage at home without assistance. ACAS provides a comprehensive medically based assessment of care needs including information, advice and referral to a range of suitable care options.
For an older person to access Commonwealth funded residential care, residential respite, Home Care Packages or Transition Care, they must first be assessed as eligible by an ACAS. Transition Care is aimed at improving independence and confidence immediately after a hospital stay.
Bendigo ACAS provides services for the Loddon Mallee Region, except the Mildura area.
Referral Process
The preferred referral method is via My Aged Care (MAC) contact centre or online. Ph. 1800 200 422.
Basic demographic information is required including full name, date of birth, contact details including address and phone number, gender, GP details, consultant details if applicable and the medical condition/reason for referral.
Referrals are prioritised depending on the circumstances of the individual.
ACAS office is co-located with Community Allied Health Services at the Havlin St East campus. Outreach offices are situated at Echuca, Castlemaine and Swan Hill.
