Mount Alexander shire Council provides a Living at Home assessment to persons in their own home. The person is actively involved in the assessment process , working with the assessment officer to develop a care plan that identifies their needs and abilities and outlines goals for the person to work towards to remain active and independent at home . The process also supports carers and in their caring role and can involve family and significant others. Consideration is made to people with special needs – financially disadvantaged ,CALD ( culturally and linguistic diverse), people of an Aboriginal or Torres Strait Islander background, people with dementia and people who are GLBTI as part of the assessment process.
Services are provided to achieve the persons goals and referrals are made for other support such as ACAS , Occupational Therapy, District Nursing , Podiatry , Social Support and other allied health. Services are provided with other agencies using a collaborative and coordinated team approach. The assessment service monitors individuals progress and will review the person as regularly or as needs change.
