East Grampians Health Service - Ararat

Services families/children from 6-12y age group with health surveillance and assessment of school entrants, referral and follow-up services, early detection and intervention on identified health problems and general health promotion, education and support to families. 

Program is available at Department of Human Services offices in Ballarat, Ararat and Horsham and outreach services.

Elmhurst Bush Nursing Centre

Provides in-home and centre-based nursing and treatment by nursing staff. Services include administering medications, wound care, medication supervision, personal care, and post hospital discharge care. The service is available by arrangement.

Grampians Health Stawell

This service provides  in home nursing assessment, clinical services, health education, monitoring and support, wound care, palliative care, continence care and assessment, hygiene assessment.  

Service available: 7.30 am - 6.30pm, Mon - Fri; limited service for weekends 0830-5.30pm, Sat & Sun.

A Community Health Nurse is available at the Health Service or may visit clients in their homes to assess and maintain health, to conduct education and support programs, provide health information resources and health promotion activities.

East Grampians Health Services - Parkland House Willaura

A service which provides in-home nursing and treatment by nursing staff.  Services include administering medications, wound care, medication supervision, personal care, and monitoring of post acute care clients.
The service is available 8.00 am - 5.00 pm, Monday - Friday, by appointment.
