Provides in-home nursing and treatment by nursing staff to assist people to remain living independently in the community. Anyone who has a health problem that affects their normal daily lifestyle or their carers is eligible. Care and treatment include: support care, rehabilitation, surgical dressings, medication, assistance with personal hygiene, stoma care, palliative care, adjustment after hospitalisation, prosthesis care, personal care alarms, specimen collection.
