Specialist Palliative Care nurses provide assessment, advice, support and care planning on a consultative basis for patients at home, in acute care facilities and RACFs, working collaboratively with primary care providers.  Monday-Friday specialist nursing service with generalist nursing backup on weekends. Limited on-call specialist service on weekends. Referrals are accepted from any source.
