MY HEALTH, MY LIFE! is a our week chronic disease self management group program, followed by individual health coaching sessions conducted over a twenty-two week period. An initial health coaching session is conducted face to face, with subsequent sessions conducted over the telephone. The program is suitable for people with chronic conditions and their carers.

This program aims to support young people who are 'transitioning' to independent living. The target group for this program is young people who have the capacity and ability to live independently but require extra support to make this transition long term.  The program is similar to the TAP service, except that the duration of support can be up to 2 years if required.
