Most nursing care can be provided in home including care which has traditionally only been available in the hospital environment. Our nurses liaise with the client, their family and other health providers to develop a comprehensive care plan which will ensure that the individual's health and care needs are met. 

Our services are provided irrespective of age, race, religion, presenting illness or social and economic status. The only criteria we have are that a person has a genuine need for care and that their needs are able to be met by the services we can provide.

Macedon Ranges Health

The District Nursing Gisborne provides domiciliary nursing care 7 days a week.  The care provided includes:hygiene assessment, general nursing care, wound management, technical care, diabetes education, continence management, holistic assessment, stomal therapy, assistance with medications, aged care and HIV Support.

This program is for persons who have experienced a cardiac event, cardiac surgery, and heart disease related episode or for those people who have risk factors and wish to prevent heart disease. Education topics Include:  •Medication Management •Symptom Management •Healthy Eating •Physical Activity •Stress Management and Relaxation

What is an Asthma clinic? 
You will have personal appointments with the asthma nurse who works with you and your GP to provide excellence in asthma care. 
Brooke Street Medical Centre's specialist Asthma Educator Nurse works with you and your GP to develop a unique plan to suit your individual health needs.

Craigieburn Specialist Consulting Suites

The day medical service operates 3 days per week. Procedures conducted at Craigieburn Health Service include trial of void, change of catheter, blood transfusions and infusions.
