Additional information
Most nursing care can be provided in home including care which has traditionally only been available in the hospital environment. Our nurses liaise with the client, their family and other health providers to develop a comprehensive care plan which will ensure that the individual's health and care needs are met.
Our services are provided irrespective of age, race, religion, presenting illness or social and economic status. The only criteria we have are that a person has a genuine need for care and that their needs are able to be met by the services we can provide.

•Wound care
•Chronic disease management
•Medication management
•Continence management
•Palliative care
•Diabetes management
•Hospital in the Home and Hospital in the Nursing Home including Intravenous therapy
•Dementia care
Referrals accepted GP, Hospital In The Home,Post Acute Care, Transitional Care, self referral, Private Health funded
