information, counselling, and education for personal, social and
health matters for patients experiencing a variety of complex
conditions.
Our Social Worker supports people with:
• Planning & problem solving for personal, family and health
issues
• Liaison and advocacy with government and non-government
services
• Information and support to access services
• Counselling including adjustment to chronic conditions;
targeted life skills training; stress/anxiety management; and
grief
• Health Coaching including smoking cessation, and transition to
cessation
• Advance Care Planning - support for Wills, EPOA, and
Advance Health Directive
• Paediatric behaviour assessment and managemen
