The Eastern Health Community Access Unit is the Defined Point of Access for ambulatory and community services provided by Eastern Health. Services include community rehab, specialists clinic and HARP. Referrals for services processed by the unit.

Austin Health - Austin Hospital

Drop-in service. Information given includes areas such as community affairs, income, assistance in obtaining other basic living essentials.
Referral can be made to health, welfare, financial counselling, housing and support agencies if appropriate.
Assistance is provided to local residents (of Glen Eira)  in need of relief from poverty, sickness, suffering, distress, misfortune, destitution or helplessness. (Please note: Emergency Relief interviews not conducted after 3.40pm)

Case Management aims to enhance your choices and improve your health and social outcomes. Following a comprehensive assessment, we develop a care plan based on your personal goals. Various support services are organised to meet your needs and maximise optimal independence. These can include linking you into services such as social, health, housing, financial support, home care and others.

Better Health Network

Facilitates the relationship between Indigenous clients and health care providers; including specialists, general practitioners, psychologists, practice nurses and allied health professionals. This includes:  Accepting referrals to assist GPs to manage patient care. Intensive assistance related to chronic disease management. Helping clients to schedule and attend appointments, accompanying them when appropriate.
