Service Description
The aim of this service is to provide education to people with diabetes type 1 and 2, so they are empowered to live a healthy lifestyle.  A Registered Nurse is available to provide education for all issues related to diabetes.

Care of Diabetes is a team effort, so we like to work closely with the person's  local Doctor, dietician, other Allied Helath workers e.g. Podiatrist, and Specialists.

The Diabetes Educator assists people to better understand the day to day management of diabetes through education (one to one, group and community), clinical care, counselling and health promotion.

Clunes Health

The National Diabetes Services Scheme (NDSS) delivers diabetes-related products at subsidised prices and provides information and support services to people with diabetes. Registration is free and open to all Australians diagnosed with diabetes. The NDSS is an Australia Government initiative, administered by Diabetes Australia. Castlemaine District Community Health is an NDSS Access Point.

In order to provide you with the best possible care, your Diabetes Team will, as a minimum, conduct / monitor the following:
* Blood tests
* Foot checks
* Blood Pressure checks
* Body Mass Index readings
* Cholesterol tests
* Medication Reviews
* Self-Care education
* Referral for Eye checks

The team aims to assist you with your understanding of diabetes and managing.

Provides information and support for people with diabetes such as: - Assistance with management of diabetes - Teaching blood glucose monitoring including: getting set up, what the results mean and what can affect the results - Providing information and explanation on various types of medications and insulin - Problem solving, to help the person take control of their diabetes
