HACC (Home & Community Care) is a government funded program designed to give families/carers a time limited break away from the usual responsibilities of caring for someone aged 0-25 years with disabilities and/or ongoing medical conditions with moderate to high support needs. Respite care provides in-house or out of home respite, personal care, assistance with transport, assistance with housework, as well as information and access to other programs.
The Specific Home Care Program provides skilled workers employed with Hobsons Bay City Council who are police checked, trained and experienced in working with children and young people (0 to 25 years) with disabilities and/or ongoing medical conditions. 
This service is available seven days a week during daytime, evenings or weekends . Bookings are necessary and can be made on a regular and/or occasional basis.

Must be eligible under Home & Community Care funding. Provision is based on assessed needs.

Aims
To provide support in the home for children with disabilities and respite for the carers of these children.

The Social Work Service assists patients of Werribee Mercy Hospital and their families with the transitiion between hospital and home.  Social Workers can help by:  referring patients and families to necessary resources and support in the community, coordinating the discharge plan from hospital, providing supportive counselling, facilitating family meetings, educating patients and families on the residential care process, helping patients expand and strengthen their network of social supports.
Social workers can provide information and support on:  accommodation/housing finances and income support, local community services, grief and loss, education and employment, enduring power of attorney and wills, aged care services, disability support, mental health, material aid, counselling services, drug and alcohol support, parenting and family relationships, domestic violence support and carer support services
Social workers also assist women during the antenatal period of their pregnancy.
The Social Work Service does not provide a counselling service to people who are not current clients of Werribee Mercy Hospital

Phoenix Westgate Migrant Centre

Assists ethnic communities through direct service delivery, advocacy and community development. The Worker provides information, referral, counselling and support to assist newly arrived migrants with special needs to gain access to basic services such as education, training, income support, health and language services. The Worker also identifies and meets the settlement needs of migrants and ethnic communities, analyses government policies and provides advocacy on policy issues.

Community Care services for people under 65 that require assistance maintaining or regaining their independence to live safely in their homes, and to actively participate in their community. The program assists families and carers in their caring role.
