Provides a home nursing service including, short term care after discharge from Hospital, supporting clients to manage medications, chronic disease, care for frail aged and disabled clients, End of life care and carer support. Wound care and nursing assessments and referrals are also provided.

Cowra Community Health Centre Palliative Care Service

Provides specialist medical palliative care. Care is provided within the unit by a palliative care specialist and medical registrar. Medical staff are rostered between 8.30am and 5.30pm and are on call after hours. All patients must be assessed by the palliative care specialists before being admitted into the Clare Holland House Inpatient Unit.
