Cogg Youth Development Unit

Provides cultural support and advocacy for Aboriginal and Torres Strait Islander patients across all inpatient and outpateint services. Services for patients and families include: providing emotional, social and cultural support, attending appointments with patients when requested, linking patients with appropriate external services, working closely with hospital staff to ensure that Aboriginal patients have a voice in their care.

Bacchus Marsh Community Health Centre

Service is aimed at clients with early stage chronic disease or risk factors for the development of chronic disease. Works in partnership with the client, family/carer, doctor and other health professionals to support the management of the client's health condition. Provides support and assistance to make behaviour changes to achieve goals as well as considering referrals to other services and providing general information about managing health conditions and risk factors.

Patient Representative at each hospital is available to assist patients, their relatives and friends, to resolve any problems they may have with hospital services. Complaints are an integral part of the quality process of the hospital and provide the opportunity for the Executive team to review and improve hospital processes, thereby monitoring and enhancing the delivery of quality health care.

Ensures the safe and effective discharge of ED patients who are medically cleared for discharge, but are screened to be at risk of not managing in the community.  Needs assessment includes: • Whether the patient lives alone • Whether they are likely to have self care problems • If they care for other people • If they already using community services  An action plan is then implemented to address each of these issues in a timely and appropriate manner in order to ensure a safe patient outcome.

The service provides you and your family: 
* emotional, social and cultural support
*a connection between you and non-Indigenous staff 
*assistance through the hospital system, including relevant referrals to other agencies
*Koori Maternity Services (KMS) works with the Midwife to support Aboriginal and Torres Strait Islander women and their families in their pregnancy journey.
