The program has a focus on supporting clients with chronic conditions and preventing hospital re-admissions related to chronic diseases.

The psychologist can assist a range of client groups with a particular focus, but not limited to:
• individuals to make lifestyle changes to maximise health and functional outcomes,
• addressing emotional and behavioural factors related to adjustment to chronic conditions or injury,
