The EWHS Central Intake is the gateway to all of EWHS community based services. On the intital contact all consumers will speak with a Community Health Nurse, who will assist the consumer to identify the consumers health priorities.

This includes:  Accepting referrals to assist GPs to manage patient care. Intensive assistance related to chronic disease management. Helping clients to schedule and attend appointments, accompanying them when appropriate.
