The Home Telemonitoring (HTM) service provides long distance telemonitoring of patients with a chronic disease. The Home Telemonitoring service is currently accepting referrals via Community Health Intakefrom ACT Health Directorate Doctors, Allied health staff, Nurse Practitioners, and Clinical Nurse Coordinators.

Chronic Disease Management Unit (CDMU)

The Chronic Disease Telephone Coaching Service (CDTC) is a telephone based support service provided free of charge. The program will help people with chronic diseases improve their ability to self manage their condition.

We are a small practice of GPs who specialise in personalised chronic disease management and health promotion. 

The Know You Co

The Chronic Care Program provides clinical support, education and care coordination (for those clients who require it) for frequent users of the acute care sector who have chronic heart failure and/or chronic obstructive pulmonary disease and/or Parkinson's disease. Eligibility criteria applies and referrals are required for inclusion in the program.

Canberra Hospital - Chronic Care Program

Hospital in the Home (HITH) allows people to be Canberra Hospital inpatients under the care of a Specialist, but in the comfort of your own home. The home service component of HITH runs 24 hours a day, seven days a week. The infusion centre is open from 8:30am - 5:00pm Monday to Saturday. Each Wednesday it opens at 9:30am.
