Western Health - Sunshine Hospital

Monday afternoon & Thursday morning (41 week clinic)

Clinic provides - care and management of women who are 41 weeks gestation and over.  Mid wife care to GP - shared care women at 36 weeks.

Hazeldean Nursing Home is a  40 bed high level residential care facility, including four single rooms and nine shared rooms with ensuites each with four beds.

Hazeldean has 24-hour nursing cover, and local General Practitioners provide medical care to residents. Local pharmacies in Williamstown ensure services 24 hours a day.

The close proximity of The Williamstown Hospital means that some hospital services are provided at Hazeldean, including social work.

Hazeldean also has a visiting podiatrist and physiotherapist, as well as a comprehensive activities program providing lifestyle and leisure activities. Residents are mainly local Williamstown and district people and are encouraged to remain active within their community. Christian religious services are held at Hazeldean fortnightly.

Hazeldean welcomes visitation of residents by volunteer groups offering pet therapy. Presently, Marmalade is the facility cat, having adopted the facility in 2001. 

The nursing home has a core group of volunteers who provide musical entertainment, assist with bingo and provide company for outings. The facility has access to the residential aged care bus for regular outings organised by the Activity Coordinator.

Provides the vital link for clients requiring ongoing community nursing care. Works to achieve best health outcomes for clients via the following processes: - Providing community focussed nursing assessments for patients - Collaborating with hospital staff for discharge planning. - Participation in family case conferences - Collaboration with other Health professionals to arrange other community services as needed 

Each Midwifery Team is responsible for the pregnancy care of the women allocated to their Team. This means that women will receive their care from the same team of midwives throughout their pregnancy (antenatal, labour, postnatal care). 

Each team provides antenatal care in community clinics as well as hospital. Patients enrolled in shared maternity care may attend a community clinic for most of their hospital visits. Women enrolled in Shared Maternity Care who would like to attend a community clinic for their hospital visits should phone 8345 1683 (Monday - Friday 9.00 – 4.30) and speak to the Shared Maternity Care Coordinator to arrange this.
