Post Acute Care Services
Supports patient's recuperation after a hospital episode, via the provision of individually tailored packages of community based care services.  The program promotes health and independence whilst providing a vital link to ongoing community support.

Services offered will depend on need and community availablity, such as:

-- Home care for housework, shopping, meal preparation

-- Personal care for showering and dressing

-- Nursing care services

-- Allied Health services

-- Transport services

Maryborough District Health Service

Patients who require support or assistance linking with community services following hospitalisation OR require assistance organising respite, transitional care or residential care.

Service Restriction: No Emergency Department

St John Of God Bendigo Hospital

Provides services in emergency, maternity, women’s health, medical imaging, pathology, rehabilitation, community services, residential aged care, psychiatric care, community dental, hospice, palliative care, cardiology, cancer services and renal dialysis to the people of the Loddon Mallee region. We offer subacute services including inpatient and outpatient rehabilitation, a regional psychiatric service, residential care, specialist clinics, dialysis and a range of outreach services.

Dunolly Hospital and Nursing Home

Daylesford Hospital provides general medical, surgical and sub-acute care including transition, dialysis, palliative and restorative care.

Discharge Planning
The Kyneton District Health Service offers a range of services including Medical, Surgical, Dialysis, Urology, Dermatology, Plastic Surgery, Orthopaedic, Paediatric, Obstetric, Gynaecology, Palliative Care, Aged Care, Emergency Care and Acute Care.

The goal of discharge planning is to provide a smooth transition between the hospital and community.  The goal is to ensure good communications between patients, their family or carer and community services.

Inglewood and Districts Health Service - Inglewood

Reception hours are 8:00am-4:30pm weekdays.

Acute medical and surgical hospital services offer accident and emergency care and inpatient and outpatient treatment in pathology and radiology.  
Other services include a diabetes educator, medical imaging and the post acute care program.
