Laura and District Hospital

Acquired Brain Injury Case Management assists people with a moderate level acquired brain injury to remain living at home.  A Case Manager provides a single point of contact for the person and their family to deal with the service system.  Case managers work one on one with clients, families and carers to identify needed services and supports for the person.  
 
Who Can We help?
The target group for this service are people aged 18 years and older who have:

-- a diagnosed ABI,  and 

-- who are non-compensable, and

-- who require moderate assistance to maintain independent living

LYNDOCH LIVING INC

While family carers can rest assured that their loved one is in the 24 hour care of experienced and qualified staff, those in our care will enjoy a holiday all of their own. While in residence they’ll have the company of a lively community, nutritious meals which cater for special diets, and a wide range of meaningful activities to participate in at their leisure. If you are coming to us from hospital we can also easily arrange any required follow up physiotherapy, or other allied health support.

Provides case management, education and counselling for patients and their families.  The service is suitable for people aged 6 - 65 years who are experiencing difficulties returning to family, work and other roles following brain injury (including stroke).

People with mild ABI are contacted by the Clinic 2 - 3 weeks after the incident to screen for any ongoing issues.  The ABI Clinic involvement may last from one initial screening phone call, to ongoing complex case management for more seriously brain injured clients.

Central Intake is an access and referral management service providing consumers; carers and patients with a single point of entry to those programs within the scope of Community Programs as well as facilitating navigation of the broader community service system.  Programs: Post Acute Care; Hospital Admission Risk Program; Grampians Regional Continence Service; Cognitive, Dementia and Memory Service; Allied Health; Rehab in the Home; Grampians Regional Palliative Care Team; Planned Activity Groups; Linkages; Aged Care Assessment Service and Carer Respite and Support Services. Central Intake is an access and referral management service providing consumers/carers/patients with a single point of entry to those programs in the scope of Community programs as well as facilitating navigation of the broader community services system

Programs includes information, secondary consultation and training, short term case management, brokerage to support short term intervention, ABI/Alcohol and other drugs case managment in Greater Shepparton. Offices are also located in Benalla, Cobram, Seymour and Wodonga.

National Respite for Carers Program.
Community Interlink is funded to deliver the National Respite for Carers Program which aims to enhance the quality of life of older people and their carers.
The focus is on supporting the primary carer, defined as the person (family member, friend, neighbour) who provides regular and sustained care and assistance to another person without payment for their caring role, other than a pension or benefit.
Respite can be offered through Personal Care, In Home Respite, Day Respite or Community-Based respite. The assistance has to be ongoing, or is likely to be ongoing for at least six months. (No short-term or one-off services.)
Community Interlink can offer advice and support on respite options for you and your carers.
