The Kyneton District Health Service offers a range of services including Medical, Surgical, Dialysis, Urology, Dermatology, Plastic Surgery, Orthopaedic, Paediatric, Obstetric, Gynaecology, Palliative Care, Aged Care, Emergency Care and Acute Care.

The goal of discharge planning is to provide a smooth transition between the hospital and community.  The goal is to ensure good communications between patients, their family or carer and community services.

Castlemaine Health is part of the Victorian public hospital network. We deliver a range of health services to people living in and around the Shire of Mount Alexander. Dhelkaya Health

Daylesford Hospital provides general medical, surgical and sub-acute care including transition, dialysis, palliative and restorative care. 

Daylesford Health, Central Highlands Rural Health

Reception hours are 8:00am-4:30pm weekdays.

Acute medical and surgical hospital services offer accident and emergency care and inpatient and outpatient treatment in pathology and radiology.  
Other services include a diabetes educator, medical imaging and the post acute care program.

24 hour Urgent Care Centre staffed by registered nurses. Medical staff on-call from 10:00am – 10:00pm daily. 

Kilmore District Health

Sunbury Day Hospital

Patients with a surgical booking are contacted by phone for health assessment, anaesthetic risk assessment and discharge planning.

Pre Admission Clinic staff use the patients health questionnaire as a guide and the patient may be booked into the clinic for an appointment with the Pre-admission Clinic nurse/anaesthetist.
Anti-embolitic stockings are fitted in Pre Admission Clinic.

Djerriwarrh Health Services - Bacchus Marsh

Melton Health

Patients who require support or assistance linking with community services following hospitalisation OR require assistance organising respite, transitional care or residential care.

Service Restriction: No Emergency Department
