To provide skilled nursing care to a patient in their home which aims to improve the health status and maximise the indpendence of clients and their care.

Services Provided:

Community Health Nurse provides initial contact and follow up for health needs, health services, health information, health promotion activities as required by individual members of the community or groups.

Services Provided:

Information and support for individuals and/or families to improve skills in Asthma Management.  Education in Emergency Asthma Management for goups - sporting, workplace, childcare.

Services Provided:

The Well Women's Clinic is run by a Women's Health Nurse for women with no abnormal symptoms.  The Well Women's Clinic offers pap testing, breast self examination instruction, pregnancy testing and information on contraception, menopause, continence, osteoporosis, sexually transmitted infections, and fertility awareness information.

Services Provided:

Cardiac rehabilitation is an eight week education and exercise rehabilitation program, facilitated by a multidisciplinary team of health professionals including Cardiac Rehabilitation Nurse and Physiotherapists. 
Prior to starting the program a one hour Pre-Program assessment is required.
Commencement of this program is recommended 2-3 weeks following discharge from hospital.

Services Provided:

Pulmonary rehabilitation is a six week education and exercise rehabilitation program facilitated by a multidisciplinary team of health professionals, including a respiratory nurse and physiotherapist.
Pulmonary Rehabilitation is held on Tuesdays 1-3pm
Consumers who complete Pulmonary Rehabilitation may choose to continue regular maintenance exercise in a community based Strength Training Group.

Service Provided:

The Heart Failure Program provides comprehensive assessment, Care Planning, Self Management Education and Exercise Program, Community linkages and ongoing monitoring and review.

currently 30 beds plus 10 dementia specific beds plus1high level care respite beds.
