Seymour Health

BHS Palliative Care consists of a 20 bed inpatient unit, providing multi-disciplinary care to patients with a life-limiting illness, and for whom the primary focus of care is maximising comfort.  Patients are admitted for symptom management, terminal care, convalescence and occasionally for family respite.

Referrals to the Palliative Care Unit should include the Palliative Care Clinical Referral form available above.pan>

Broadmeadows Hospital

Provides a Palliative Care Suite with full ensuite facilities for palliative patients. There is also a bed settee and kitchen facilities for carers and family members. Palliative Care is an inpatient service providing 24 hour nursing care in conjunction with support from family and care givers. 

Central Highlands Rural Health - Kyneton Health

Provision of care to clients with a life limiting illness. It is a government funded service where experienced Registered nurses provide high quality holistic care. Care includes: - Assessment and management of management of difficult symptoms such as pain, nausea, constipation, etc; - Emotional, spiritual and psychological support to client and family including bereavement visits; - Liaising and referring to other providers ensuring best practice; - 24 hour on call support service.

Holloway Aged Care Services

Melbourne City Mission - Palliative Care

A ten bed inpatient palliative care unit providing multidisciplinary care to patients with a progressive, life-threatening illness. Patients are admitted for symptom control, terminal care and respite care. The multidisciplinary team includes a social worker, occupational therapist, physiotherapist and pastoral care worker.
