mecwacare North West Metro Service

mecwacare's Home Nursing and Care North West Metro Service supports clients of all ages and abilities, including older people, those with illnesses such as multiple sclerosis or dementia, and healthy people with an acquired brain injury or an intellectual disability. We support our clients to remain in their own homes for as long as they wish. Each health and wellbeing service is integrated so care is tailored to meet client needs, yet is flexible as circumstances change. 
We can provide the following care and services to
assist you in your home:
• Home Nursing Services including medication and wound management, post-hospital care and enteral therapy
• Respite and Overnight Care to help relieve the stress of being a carer, including 24-hour live-in care for clients who may require a higher level of support
• Personal Care and Home Help 
• Meals, Companionship, Transport and Travel
• Specialised Care and Private Care

Niddrie Medical Centre

The RDNS Liaison Service provides the vital link  for clients requiring ongoing community nursing care.  RDNS Liaison Clinical Nurse Consultants work with hospital staff and other health care services and providers to achieve continuing best health outcomes for clients via the following processes:     
•	Providing community focussed nursing assessments for patients 
•	Collaborating with hospital staff for discharge planning.  This includes discussion of patients on-going needs and ensuring that all equipment, supplies and information has been provided prior to discharge. 
•	Participation in family case conferences 
•	Coordinating admission to RDNS including the scheduling of home visits by specialist and general nursing staff 
•	Collaboration with other Health professionals to assist/arrange other community services as needed eg. Home Help, Meals on Wheels.

When the Liaison CNC is not in attendance, please refer to the information folder 'Referral to RDNS' located on all wards and in major departments
