A service which provides in-home nursing and treatment by nursing staff.  Services include administering medications or injections, wound care, medication supervision, personal care, and monitoring of post-acute care clients.

West Wimmera Health Service - Nhill Campus

Provides a time limited short term intervention for four weeks post discharge which is designed to assist patients to recuperate following an acute hospital admission and to facilitate their independence, or transition to continuing care when required. This care may include, home help, personal care, delivered meals, respite care and home nursing.  The program provides services to clients who meet the Home and Community Care criteria and are not covered by other programs. 

Provides nursing care in the home; treatments include medications, palliative care, wound management and assistance with personal care.  Post-acute care is also offered following an acute hospital admission to assist in the transition back into the community.

Weekend services are only available from the Warracknabeal site.

Rural North West Health - Warracknabeal Campus

Community Health nursing services are available during centre hours.  Services may include outpatients service, dressings, patient monitoring, E.C.G.'s, assessment of minor injuries and suture removals.

Rural North West Health - Beulah Campus

A service which provides in-home nursing and treatment by nursing staff to assist people to remain living independently in the community.  
The District Nurse provides care in the home for people with long- or short-term illnesses.  Anyone who has a health problem or wellbeing problem that affects their normal daily lifestyle or their carers' is eligible.

Hospital with 5 acute beds, providing medical treatment, emergency medical treatment, community support, allied health services, and aged care.

Services include administering medications or injections, wound care, medication supervision, personal care, and monitoring of post acute care clients.  Service available daily, by appointment.
