This is a short term intervention (up to 28 days) to provide additional home-based services such as District Nursing, Personal Care, Home Care and Respite Care (in home) to assist individuals to recuperate after hospitalisation.

Access centre receives and processes all referrals for the Royal Women's hospital for Parkville and Sandringham. Able to receive electronic referrals from Argus users. Otherwise, referrals need to be faxed (Fax: 8345 3036).
