Rural North West Health - Hopetoun Campus

A service which provides in-home nursing and treatment by nursing staff.  
Services include administering medications or injections, wound care, medication supervision, personal care, and monitoring of post acute care clients.  Service available daily, by appointment.

Community Health nursing services are available during centre hours.  Services may include outpatients service, dressings, patient monitoring, E.C.G.'s, assessment of minor injuries and suture removals.

Rural North West Health - Beulah Campus

A service which provides in-home nursing and treatment by nursing staff to assist people to remain living independently in the community.  
The District Nurse provides care in the home for people with long- or short-term illnesses.  Anyone who has a health problem or wellbeing problem that affects their normal daily lifestyle or their carers' is eligible.

A Community Health Nurse provides health assessment, monitoring and information as well as facilitating health education to individuals, groups and the wider community.

The Community Health Nurse is able to provide health assessments, pap smears, womens health, mens health, diabetes & asthma advice and information, and referral to other health professionals.
The service is available by appointment.

Mallee Track Health & Community Service - Patchewollock Service Centre

West Wimmera Health Service - Jeparit Campus

Community Health Nurses at East Wimmera Health Service work in the community and provide services aimed at promoting the health and wellbeing of our communities, including Diabetes education, Chronic disease management, QUIT program and  Life Program. Community Health Nurses work one on one or in a group setting.

East Wimmera Health Service - Birchip Campus

Provides nursing care in the home; treatments include medications, palliative care, wound management and assistance with personal care.  Post-acute care is also offered following an acute hospital admission to assist in the transition back into the community.

Weekend services are only available from the Warracknabeal site.
