General Health Support, Counselling and Advocacy to promote Health and Wellbeing. The aim of the HARP Program is to ensure clients, who frequently present to the Royal Melbourne Hospital, are adequately receiving their care which responds to their needs in the community. Merri HARP services include: Heartwise (Chronic Heart Failure service); Melbourne Easy Breathers (Chronic Respiratory Disease service); Community Diabetes foot service; Diabetes co-management; Care Facilitation service.
