Hospital in the Home (HITH) allows people to be Canberra Hospital inpatients under the care of a Specialist, but in the comfort of your own home. 

The home service component of HITH runs 24 hours a day, seven days a week. 

The infusion centre is open from 8:30am - 5:00pm Monday to Saturday. Each Wednesday it opens at 9:30am.

You are eligible for HITH if you:

–	You live in Canberra or Queanbeyan;
–	You have a telephone;
–	You are self caring or have a suitable carer;
–	Your condition is clinically appropriate for home-based care;
–	Your home environment is safe for the delivery of treatment.

Canberra Hospital - Hospital in the Home (HITH)

The Chronic Care Program provides clinical support, education and care coordination (for those clients who require it) for frequent users of the acute care sector who have chronic heart failure and/or chronic obstructive pulmonary disease and/or Parkinson's disease. Eligibility criteria applies and referrals are required for inclusion in the program.  Patients may be referred by specialists, GPs or other members of the multidisciplinary team who identify a client who may benefit from any or all parts of the service.  A patient may self refer and will be assessed as eligible or non eligible based on their clinical condition. Patients who require clinical support or education from the CNCs only will be booked in to the specific clinic at a time dependent on the patients condition or as per request of the specialist cardiologist or respiratory medicine or neurologist and/or reviewed on the ward if still an inpatient and an appointment made post discharge.

Canberra Hospital - Chronic Care Program

The Chronic Disease Telephone Coaching Service (CDTC) is a telephone based support service provided free of charge. The program will help people with chronic diseases improve their ability to self manage their condition. This leads to improved quality of life and reducing the incidence of exacerbation and progression of their disease. The Telephone Coaching program includes an individualised plan to help patients achieve their personal health goals, regular support calls from the program's registered nurse, advice on ways to improve aspects of the patient's health including weight management, understanding their medication and the importance of exercise, advice on effectively communicating with their doctor and access to 24 hour, 7 day a week telephone support. a referral from any treating clinician in required for Telephone Coaching

Chronic Disease Management Unit (CDMU)

The Home Telemonitoring (HTM) service provides long distance telemonitoring of patients with a chronic disease.  The Home Telemonitoring service provides installation of a telemonitoring station in a patient's home for anywhere between 1 and 3 months,  patient education in the use of the home telemonitor to record their temperature, blood pressure, heart rate, heart rhythm, weight, oxygen saturation, blood glucose and respiratory function each day and monitoring of daily observations via the internet (Monday to Friday) by a clinician. The Home Telemonitoring service is currently accepting referrals via Community Health Intake (phone 6207 9977 or fax 6205 2611) from ACT Health Directorate Doctors, Allied health staff, Nurse Practitioners, and Clinical Nurse Coordinators. Chronic disease (such as cardiovascular disease, cancer, diabetes, asthma and arthritis) accounts for around 80% of the total burden of illness in Australia.

Chronic illnesses can compromise quality of life and lead to a gradual deterioration of health. An option for patients with chronic disease is preparation of a Care Plan. 

Care Plans drawn up by Airport General Practice doctors typically include a detailed overview of the patient's health history and treatment; and identify treatment objectives and methods, patient self-care options and the identity of other health professionals involved in their care. These health summaries can be retained by the patient for presentation at hospital or to other health professionals.

Special interests of AGP doctors include: women's health, dermatology, minor and cosmetic surgery and travel medicine.
