Provides information and support to individuals and groups regarding asthma, 
diabetes, cancer prevention and heart health and healthy lifestyle programs. 
Provides a home nursing service including, short term care after discharge from 
Hospital, supporting clients to manage medications, chronic disease, care for frail 
aged and disabled clients, End of life care and carer support. Wound care and 
nursing assessments and referrals are also provided.

• hospital bookings for birth 
• home visits for antenatal care provided by a midwife and an Aboriginal Health Worker. 
• pregnancy education

• breastfeeding support and education 
• Support up to 8 weeks after birth. 
• Contraception advice.

Aboriginal Health Workers assist Aboriginal people to have a safe patient journey through the Health Service. Aboriginal Health Workers can help with support, guidance, education and advice, language interpretations, health promotion, prevention and education, 48hr follow-up, referrals and links to other services including doctors and other community services.

Aims to promote independent living. Some of the services they can support with include - Health assessments - Palliative Care - Care co-ordination and advocacy - Health monitoring - Short term care after discharge from hospital - Diabetes education - Medication assistance and supervision - Wound management - Carer Support. Priority of care is aimed at people who are frail, aged, disabled, have a chronic illness and carers.
