Aims
PAC has two main aims:
•	to purchase packages of services on behalf of individual clients which respond to their specific post acute care needs; and
•	to promote system change in the hospital and community based sectors which supports improved discharge planning and greater continuity of patient care.
PAC funding enables the purchase of individually tailored packages of health and community care services such as home nursing, personal care, allied health services and home help following discharge from hospital. As the PAC Program provides funding for the provision of additional post acute care services as required, it acts to augment the current service system, not substitute for existing services. Post Acute Care is a short term service (usually up to 4 weeks post discharge).

This service provides assessment and management of adult inpatients presenting with disorders of communication and/or swallowing of various origins (including neurological, surgical, general medical and aged care).  Services are provided across the continuum from the acutely ill through to rehabilitation as well as the support and maintenance of swallowing and communication in degenerative disorders as appropriate.

Adult outpatients are seen only at Western Hospital for the assessment and short-term management of voice disorders.  Referrals must be made via the Western Hospital Ear, Nose & Throat Clinic.

Outpatient Paediatric Speech Pathology Services are provided to young children from birth to school entry (approximately 6 years of age) who present with communication and/or swallowing difficulties.  Problems may commonly include: unclear speech, delayed language and comprehension skills, and/or difficulty sucking, chewing or swallowing. Specialist speech therapy input is also provided to Developmental Assessment Clinic, Learning Difficulties Clinic and Breastfeeding Clinic.

Neonate and paediatric inpatients are seen as required.  

Written referrals are accepted from GPs and community agencies for children who live within the municipalities of Brimbank and Maribyrnong.
