Vinnies Centres - St Albans

Ensures the safe and effective discharge of ED patients who are medically cleared for discharge, but are screened to be at risk of not managing in the community.  Needs assessment includes: • Whether the patient lives alone • Whether they are likely to have self care problems • If they care for other people • If they already using community services  An action plan is then implemented to address each of these issues in a timely and appropriate manner in order to ensure a safe patient outcome. 

Western Health - Sunshine Hospital

The Aboriginal Health Unit is passionate and committed to providing the Aboriginal and Torres Strait Islander Community a safe and welcoming environment at all of our services. As an Aboriginal and Torres Strait Islander using our services we will provide you and your family: * emotional, social and cultural support
*a connection between you and non-Indigenous staff 
*assistance through the hospital system, including relevant referrals to other agencies
*ongoing training for WH staff including asking, as part of the admission process, Aboriginal and Torres Strait Islander identification. This allows staff to ensure that you have an opportunity to access the Aboriginal Hospital Liaison Officer (AHLO) and Koori Maternity Services (KMS) AHLO
*Koori Maternity Services (KMS). The role of the KMS –AHLO is to work with the Midwife to support Aboriginal and Torres Strait Islander women and their families in their journey through: pregnancy and the antenatal period, labour and birth, the postnatal period (the first six to eight weeks of baby's life)

Footscray Hospital - Western Health

Provides cultural support and advocacy for Aboriginal and Torres Strait Islander patients across all inpatient and outpateint services. Services for patients and families include: providing emotional, social and cultural support, attending appointments with patients when requested, linking patients with appropriate external services, working closely with hospital staff to ensure that Aboriginal patients have a voice in their care.

Werribee Mercy Hospital

Patient Representative at each hospital is available to assist patients, their relatives and friends, to resolve any problems they may have with hospital services. Complaints are an integral part of the quality process of the hospital and provide the opportunity for the Executive team to review and improve hospital processes, thereby monitoring and enhancing the delivery of quality health care. 

Williamstown Hospital

Wadja Aboriginal Family Place- Wadja Health Clinic is a weekly general medical clinic for Aboriginal children which provides medical, social, cultural and emotional assessment. The focus of the clinic is on the management of Aboriginal patients with complex health and social issues by a team.  A paediatrician will see the child and family together with an Aboriginal Case Manager with the aim of enabling a family to feel supported and culturally empowered. An Aboriginal Family Health Worker will also be available during clinic hours for further support if required by the family.  No Cost. Referrals to the clinic can be made by GPs who have assessed that a child requires specialist medical care. REFERRAL Information:   Please call (03) 9345 6111 and ask to speak to our staff.
