Merimbula Home Nursing Service

Specialist palliative care nurses provide assessment, advice, support and care planning with the generalist primary health nurse to care for patients at home and also provide advice to general practitioners.

Queanbeyan Community Health Centre 

The Community Care Program provides multidisciplinary services in the community setting. Eligibility criteria apply and information can be obtained through the Community Health Intake. Services are usually delivered in health centre clinics in the first instance unless Community Nursing deem that a home visit is required. Gazetted fees and charges may apply for some items.

Staff will discuss and assess your needs with you, and help you to explore options available through: -	Provision of direct clinical nursing care; -	Comprehensive clinical assessments; -	Teaching individuals and their carers how best to manage their daily care at home; -	Provision of health information and education; -	Monitoring of health status.
