A specialised service for the carers of people with degenerative neurological conditions. Staffed by a Community Health Nurse, Physiotherapist, Occupational Therapist and Social Worker.
It is a domiciliary service.
Home assessment and development of a individual program.
Assistance with respite planning.
Integration into community activities.
Education.
Provision of on-going support, monitoring and review.

Kingston Centre

Assisted care and housing for the aged.
Assistance provided to low income frail and at risk elderly people to access and be maintained in secure and affordable housing with support, by;
- Negotiating housing needs
- Assistance with documentation
- Acessing appropriate community support services and material aid on behalf of    clients.

The ACHA worker is available four days per week and can be contacted on the number below for a referral.  (Appointment made with ACHA worker.)
