The Continuing Care Program is a multidisciplinary team providing case management for clients with a severe mental illness aged 18 to 64.

Long term heart and lung conditions such as heart failure and asthma
Musculoskeletal conditions such as arthritis or chronic pain
Depression or anxiety
Other long term conditions that will benefit from lifestyle changes.
The Child Psychology service is available for children with difficulties in the areas of behaviour, social/emotional and/or cognitive development. Children may be referred if there are concerns about their behaviour, social skills, concentration or toileting etc.
