Ballarat Health Services - Queen Elizabeth Centre

Team includes two Palliative Care Physicians, Palliative Care Coordinator, Palliative Care Nurse Educator and Palliative Care Clinical Nurse. The role of the Team includes elements of education, collaborative strategic planning, preparation of written materials, quality improvement processes and clinical consultation.

Ballarat Health Services - Central Intake

Ballarat Hospice Care Inc

Provides a Palliative Care Suite with full ensuite facilities for palliative patients. There is also a bed settee and kitchen facilities for carers and family members. Palliative Care is an inpatient service providing 24 hour nursing care in conjunction with support from family and care givers. 

Central Highlands Rural Health - Kyneton Health

Provision of care to clients with a life limiting illness. It is a government funded service where experienced Registered nurses provide high quality holistic care. Care includes: - Assessment and management of management of difficult symptoms such as pain, nausea, constipation, etc; - Emotional, spiritual and psychological support to client and family including bereavement visits; - Liaising and referring to other providers ensuring best practice; - 24 hour on call support service.

Enhances the quality of life for clients and carers as they deal with a life-limiting illness. Supports, educates and increases the confidence of other health workers and residents in the community who assist those requiring palliative care. Castlemaine Health

The Community Palliative Care service aims to respect the rights of people with a terminal illness to live their life in the way they choose until death. This is done through the provision of holistic palliative care, acknowledging the physical, psychological and social needs of clients, families and carers.  The services aims to provide an alternative to hospital care for the terminally ill.
