The Community Connection Program at AccessCare Southern endeavours to engage people in the community who are homeless or at risk of homelessness and support them to engage with services and supports available. Anyone can make a referral to the Community Connection Program, the Case Managers who deliver the program also complete outreach in the community to engage with people who are vulnerable.

The crisis centre, located at Frankston is an entry point for individuals, families and young people who are homeless or at risk of homelessness. The centres provide assessment, information, advocacy and referral to a range of accommodation and housing services.  Assessment for partial assistance with rent in advance is also available for those on a limited income. Please note there is no crisis accommodation in the Frankston area.

Links to other supports may include services such as: family violence, drug and alcohol, health and mental health, legal, finance, employment and training.

Endeavour Ministries Andrews Centre

Homeground provides a range of homelessness support such as ConnectED, Social Housing Advocacy and Support Program, Private Rental Program, Housing Mental Program and can also provides long-term support to people with complex needs and long histories of homelesness.

ConnectED- Outreach program aims to reduce preventable use of Alfred Hospital's emergency services.

Social Housing Advocacy and Support Program (SHASP) - Supports public and social housing residents

Private Rental Acess Program - Housing-focused support for women/children leaving family violence

Housing Mental Health Pathways Program - Prevents homelessness for people leaving hospit
