Facilitate Occupational Therapy Services opened in 2014, and provides OT services across the Illawarra and Shoalhaven. We are a team of 7 experienced Occupational Therapists, providing high quality services to clients of all ages. We provide our services via Home, Work and School visits, or in our Woonona Clinic. Our intervention aims to improve client safety and independence during mobility and functional tasks, while maximising client independence and quality of life.
Additional Services Provided: Mobility assessments; Functional assessments; ADL retraining; Falls prevention assessments and education; Disability, paediatric, geriatric and rehabilitation services; Hand therapy; Wheelchair and scooter prescription; Environmental modifications; Equipment prescription; Home modifications; Manual handling and work safety assessments; Mental health; Occupational Therapy Reports; Applications to Funding Organisations (EnableNSW, DVA, ADHC, NDIS).
