SACS program provides person centred, interdisciplinary care support via flexible service delivery in a range of settings. The team includes allied health professionals and nurse with group sessions includign cardiac and pulmonary rehabilitation as well as joint replacement groups. The program is goal orientated and provides time limited episodes of care.
