Additional information
The service provides care co-ordination of Health and Community Services to better support and maintain people with chronic health conditions, Chronic Obstructive Pulmonary Disease (COPD), Chronic Heart Disease (CHD), Diabetes Mellitus. Key components involve the client, carer, clinicians, care co-ordinators, the client's GP and other health professionals with timely intervention, care planning and self management education. The service is available between 8.30am to 5pm, Monday to Friday.
