The Diabetes Transition Support Program at Westmead Hospital was established in 2001 to assist young adults aged 15-25 years, with type 1 diabetes in making a successful transition from paediatric health services to adult health care.
The service accepts all young adults in this age range with pre-existing or new diagnosis of type 1 or type 2 diabetes. Young people with CF related diabetes may also be referred to this clinic.
