The Social Work Service assists patients of Werribee Mercy Hospital and their families with the transition between hospital and home. Social Workers can help by: referring patients and families to necessary resources and support in the community, coordinating the discharge plan from hospital, providing supportive counselling, facilitating family meetings, educating patients and families on the residential care process, helping patients expand and strengthen their network of social supports.
