Our Primary and Community Health Nurse Service aims to promote independent living. It can support you with Health assessments, Palliative Care, Care co-ordination and advocacy, Care after hospital discharge, Diabetes education, Medication assistance, Wound management, Carer Support. This can assist with a person's quality of life, and help them to stay at home longer and more safely. Our priority is people who are frail, aged, disabled, or have a chronic illness, and their carers.
