Break Thru's Home Based Outreach Support (HBOS) program provides outreach support to people with a desire to live independently in the community.
Planned support is available for a negotiated period of time through a key worker, between the hours of 9am-5pm, Monday – Friday. This service offers two levels of support, Standard and Moderate. An Independent Support Plan, created for each client, will determine his or her personal requirements. Standard HBOS provides up to two hours support a week, whilst Moderate HBOS provides up to six. However, support hours can be increased for short periods of time, for example when a person initially moves in to independent accommodation or at times of personal crisis.
Support is flexibly applied to meet individual choice and needs, and is provided to individuals within their own preferred community setting, whether that is in their home or other community environment. The emphasis is around consolidating and reinforcing an individual's strengths, abilities and potentials. A key focus of this service is the linking of people with appropriate community supports and developing and/or maintaining skills and abilities around: independent living and home management; problem solving and motivation; personal and emotional support; social relationships; facilitation of educational, vocational and personal development activities and pursuits; and recreational and leisure activities.
