GV Health Continence Service is a community based service. The Continence Service provides nursing assessment, advice and management strategies to assist clients and carers regarding bladder and bowel control. Three sub-programs of the Continence Service are: HACC Continence Nurse Advisory Service Continence Clinics and Continence Support Service (for children with a disability between the ages of 5-15 years) All programs offer information and advice on suitable aids and appliances and how to access them. They provide the most up to date information and education to clients, carers, health professionals and direct care staff. Referral process: 5832 3100 to make a referral, including the client's estimated date of discharge (for inpatients) and reason for referral. Referrals can be made by the individuals themselves, health professionals, general practitioners, relatives, friends or care providers, either by telephone or by referral forms i.e. SCTT (Consumer information, Profile – living and caring arrangements, Profile - functional assessment, Profile – Functional Summary and Referral including past medical history and medications, and consumer consent.)
