Prince of Wales Community Physiotherapy provides care to community-dwelling individuals who are housebound or unable to access traditional outpatient services. The service also enables appropriate onward referrals to meet complex needs.
Care is delivered under both state-funded programs-primarily supporting patients following hospital discharge to facilitate safe return home, maintain independence, and prevent hospital readmission -and Commonwealth funding through the Commonwealth Home Support
