Offer reviewing medical history, medications, functional status, cognitive status, mood, behaviours of concern, and advanced care planning. We often get referred for behaviours of concern, cognitive diagnosis, optimisation of medical issues, falls, medication or psychotropic review. We aim to optimise and improve patients’ health and functional status, speak to families, and write a comprehensive plan for the GP and the residential care facility.
