This service will be responsible for coordinating evidence-based health promotion intervention programs such as HEAL and Healthy Aging, that address the prevention of chronic disease, with a focus on physical activity and falls prevention in the populations of Western New South Wales. Clinics will largely be grouped based and provided face to face, virtual or else hybrid with support from Allied Health Assistants on-site, Rural Health Graduates, Health Promotion officers and Administrations.
