Clinical Nurse Consultant Palliative Care provides inpatient and outpatient clinical care, service coordination and education for patients, carers and staff. Care is co-ordinated with the Palliative Care Occupational Therapist, Palliative Care SCMO, General Practitioner as well as other health and non health providers as required.
